To make an appointment at Polinsky Medical Rehabilitation Center, we need a therapy order from your doctor, specialist, nurse or social worker. We can help you obtain a physician’s order.

Our center, located at 530 E. Second St. in Duluth, Minn., is open from 7:30 a.m. to 5 p.m. Monday through Friday.

Polinsky Medical Rehabilitation Center accepts all insurance, Medicare and Medicaid. Many health plans require referrals from your primary care provider. Some health plans must pre-approve each visit to Polinsky. Please contact your health plan to learn more about the need for a referral or prior approval for therapy services.
